I've actually seen this at other Japanese companies too: exhibitions of failures and accidents in an attempt to learn from the past. It feels so Japanese. Do other companies around the world do this?
A museum of failures is a wonderful thing
Some semesters, I run a "best error" or "best error" contest for my students: most perplexing, most educational, .... It creates a fun way to learn from mistakes, and a reminder that mistakes are par for the course.
